GUIDE (Guiding an Improved Dementia Experience) is a nationwide program designed to improve quality of life for people living with dementia and to support family and unpaid caregivers through coordinated care and practical services.

Executive Home Care works with approved GUIDE organizations to help eligible clients access benefits—including covered respite care—so families can get relief while their loved one continues receiving compassionate support at home.

How Executive Home Care Supports GUIDE as a Referral Partner

Executive Home Care does not determine final eligibility for GUIDE. Instead, when a client appears to qualify, we help connect the family to a participating GUIDE organization for official screening and enrollment.

Once the client is approved and enrolled, Executive Home Care can provide in-home respite care services (when available in your area and aligned with the GUIDE partner’s process), so caregivers can rest, recharge, or handle personal responsibilities—while their loved one remains safe and supported.

What to Expect If You Enroll

GUIDE is designed to support the person living with dementia and the caregiver through ongoing engagement.

1) Enrollment assessment (required)

A comprehensive assessment is completed by the GUIDE organization to build a care plan. This is typically done by phone or video and takes about 60–90 minutes.

2) Monthly check-ins (required)

Participants generally have a monthly Care Navigator check-in (often by phone, about 15–30 minutes) to monitor needs, coordinate resources, and support the caregiver.

3) Ongoing care coordination

The GUIDE team may coordinate with providers and help identify helpful services. Participation does not require switching providers, and families can decline recommendations.
